Approval and Forms
The forms necessary are based on the type of project.

HS: Review the ISEF rules for High School students
After review: if you are in doubt as to which forms or approvals need to be obtained, contact SARSEF SRC (src@sarsef.org).
All K-8 projects which require SRC approval before the project begins must submit the forms to their school level SRC. You do NOT need to submit these forms to SARSEF, but do save them in case there is any question of whether they were completed.
If the School SRC is in doubt, they may submit the forms to the SARSEF SRC:
SARSEF SRC, 7380 E. Snyder Rd., Tucson AZ 85750 (allow 2 weeks)
- Grades K-8: SARSEF has one SRC/IRB Approval form, plus the research plan which must be attached. Include all safety precautions that will be in place (for the safety of the student and the subject, depending on the project). IF Human subjects are involved in a study, please complete the K-8 Informed Consent Form (see the Informed Consent link under Student Rules)
All High School projects which require SRC approval before the project begins
must submit the forms to either their School SRC or SARSEF SRC:

- Grades 9-12: High School Students must prepare the SSP defined ISEF forms as they apply to their projects. Click on the ISEF FORMS. to reveiw the rule book and to obtain a pdf file of the required forms.
Forms 1, 1A and 1B are required for all projects Grades 9 to 12:ISEF FORMS

Form 1 - Checklist for Adult Sponsor/Safety Assessment Form
Form 1A - Student Checklist / Research Plan: Individual and Team Projects have separate forms
Research Plan: A complete research plan must accompany the 1A form. This form gives the specific items that must be addressed in a research plan.
Form 1B - Approval Form This form is signed by the student, parent/guardian and SRC if required.

Forms 1C to Form 7 are only required for those projects to which they apply.ISEF FORMS

Form 1C- Regulated Research Institutional/Industrial Setting Form (Completed after experimentation)
Form 2 - Qualified Scientist Form (Prior to experimentation)
Form 3 - Risk Assessment Form (Prior to experimentation)
Form 4 - Human Subjects Form and Informed Consent (Prior to experimentation)
Form 5A or 5B - Vertebrate Animal Form (5A or 5B) (Prior to experimentation)
Form 6A - Potentially Hazardous Biological Agents Form (Prior to experimentation)
Form 6B - Human & Vertebrate Animal Tissue Form (Prior to experimentation)
Form 7 - Continuation Projects Form (For projects spanning multiple years) (Usually completed after completion, If a project is in the same line of research, or can not stand alone without the previous years research, complete this form.)
